Registration                                                                        The practice will cease to accept new registrations for a period of 5 months from Monday 6th November 2023.If require to register with a GP, Please register with Church Street Surgery from Monday 6th November.    To register permanently with a general medical practice (GP practice) you'll need to complete a registration form. You can download this registration form from NHS Inform website, complete it, and then email it to the GP practice. nhsh.gp84256@nhs.scot
